  • Up to 12 months (co-terminus with contract) 

Basic Requirements

  • Completely filled out application form 
  • 2 valid government-issued IDs 
  • 2 pcs 2×2 picture 
  • Proof of billing (electric bill or water bill) 
  • Proof of residency* 

Major Requirements

  • Updated ORCR 
  • Certificate of employment and pay slip for employed 
  • Business permit or proof of business transaction for self-employed
